Your Rights as a Health Care Consumer

In accordance with House Bill 4224 (89th Regular Session) and Texas Health & Safety Code §181.105, please find the following information on how to access your records and how to contact appropriate agencies:

Requesting Your Health Care Records

You have a right to your individual records.  Clients and former clients may request copies of their health care records by mail.

To request records, submit a written request that includes:

  • Your full name and date of birth
  • Contact information (phone number, email address, and mailing address) We have to be able to contact you after your request.
  • Specific records requested
  • Please include dates of sessions.
  • Preferred delivery method (secure email, mail or in person pickup)
  • Signature and date

Please keep in mind the following:

Requests made on behalf of another person require documentation of legal authority (e.g., guardianship or medical power of attorney).

Identity verification may be required.

Sessions involving more than one person require the written request and consent of all parties.

Sessions involving minors may be subject to additional processes/requirements depending upon current court orders or legal situations.

There is generally a fee for obtaining records depending upon the volume and amount.
